Here's one -- when an author asks in a story: "What just happened?"  Because if the author doesn't know, how the hell does she expect the readers to know?  Even worse: "What did just happen?"
Sometimes this goes with the occasions when the reader is wondering why something happened, because there's no motivation for the protagonist to possibly act like that.  As in, no setup for the character to act out of character, because you don't even know what would be in character.  Perhaps some of this comes from slash fiction, in which the protagonists are already more or less developed, and the readers already have expectations about how the characters will act.  Perhaps it's just that the writer can't write.

Sheesh, I'm cranky tonight.
